HOW DO I SCHEDULE AN APPOINTMENT?

There are several ways to schedule your first appointment. You can contact us by emailing us, or calling us at  260.570.4515

CAN I PICK WHICH THERAPIST OR PRACTITIONER I WANT TO SEE?​

We recognize that therapy is a very intimate and sensitive process with your ability to feel comfortable with your therapist being key.  If you have a preference for a specific therapist please let us know and we will do our best to accommodate you.  At times, we may recommend a different therapist based on why you are seeking counseling.  This is because each of our therapists has distinct specialties and training that might be more helpful in specific circumstances.  Ultimately, we will always allow you to make the final decision about whom you work with. 

WHAT METHODS OF PAYMENT DO YOU ACCEPT?​

We accept cash, private insurance, & credit cards for payment.

WHAT IF I DO NOT HAVE INSURANCE?​

We also accept cash and credit card for payments.  We do offer a sliding fee scale for those who cannot afford to pay for services.  We can assist you in estimating our fee based on the sliding fee scale prior to your first appointment.

WHAT IF OAKTREE GUIDANCE & WELLNESS CENTER
IS "OUT OF NETWORK" FOR ME?​

Some of our clients may be covered by an insurance that we are not networked with.  Occasionally we can attempt to become paneled with this insurance company so that we are "in network".  On other occasions you may choose to work with us with the understanding that you will be responsible for a higher copay then if you were seeing another clinician who was "in network."

WHAT DO I NEED TO BRING TO MY FIRST APPOINTMENT?​

Prior to your first appointment, please complete the necessary forms & return them to the office or we may connect the forms online to your patient portal.  At your first appointment please bring a copy of your insurance card.   Plan on arriving at our office at least 10 minutes prior to your scheduled appointment time, to fill out any forms that may be incomplete.

WHAT IF I HAVE TO CANCEL MY APPOINTMENT?​

Please call us as at your earliest convenience to cancel scheduled appointments.  A fee of $100 may be billed for appointments that are not cancelled before 24 hours prior to your appointment time.
